The admission process. |
| In order to ensure the most appropriate level of care to each and every resident, all potential admissions require a nursing assessment. This assessment is obtained either in person, over the phone, or via FAX. After completing the nursing assessment and being accepted for admission, each potential resident must obtain their physician's order for admission to a skilled nursing facility. |
Coming from home? |
If coming from home, a nursing assessment can be completed in person or over the phone with the potential resident or appropriate representative. Within 7 days prior to admission, an incoming resident must visit her/his primary care physician for a physical examination and orders for her/his plan of care.

Coming from hospital?

| When transfering from a hospital, a social worker will assist with the admission process by gathering appropriate information to complete the nursing assessment. Typically, the assessment information is faxed directly to us by the hospital social worker. The attending physician must complete orders to transfer to a skilled nursing facility. |
