We provide long-term memory care in a skilled nursing environment that closely mirrors the experience of living in a private home with family caregivers. Our Household model of care is a welcome replacement for the traditional nursing home! At present, though, we are not able to serve those who require a secure memory care residence. That will change in fall 2014 with the opening of Episcopal Church Home – The Gardens. Its two 10-resident Green House Memory Care Homes will be the first in Minnesota to be designed around the trailblazing Green House Model of Care.

In many nursing homes that serve residents with memory issues, a very important element has disappeared from the culture: HOME. We believe that our seniors deserve a life that is as close to home as possible. Our unique Household model of care creates “HOME” where residents experience joy, dignity, and comfort in their daily living.

Episcopal Church Home offers long-term memory care in a community of six distinct Households. Our Household model provides the best personal, health and medical services any resident will need. Each day is informal, like a day in one’s own home, and results in a truly rewarding and satisfying life for residents and staff alike. It’s not just “home-like,” it’s home!

Episcopal Church Home accepts admissions 24 hours a day, 7 days a week. Daily rates are based on level of care required for the resident. We accept Medical Assistance, Private Long Term Care Insurance, HMO, Medicare and private/personal funds.

Everyone eats the same meals at the same time. Residents eat what they want, when they want. Just like at home.
Everyone is awakened at the same time. Residents wake up when they want to. Just like at home.
Low staff-to-resident ratios. Rated 5 of 5 stars for staffing ratios by the Minnesota Department of Health and Human Services.
Rotating staff, often supplemented with outside personnel from nursing pools. Consistent staffing. No outside help from nursing pools. Residents and staff become family.
Staff plans daily resident activities. Community Circles of residents and staff direct their life in each Household.
The “med cart” comes clattering down the halls at specific times. Medications are stored securely in each resident’s private room and dispensed at the best times for each individual.
