We are the Twin Cities’ first independent transitional care facility to offer all cardiac patients a Heart Health & Healing Program in cooperation with United Hospital and the John Nasseff Heart Center. The program includes patient education on diet, exercise, weight monitoring, medication and supportive community resources.
