Heart Healthy Program

 

Heart Healthy is a hospital-developed “Total Heart” program that has proven to reduce readmissions by up to 35%.

 

Our Heart Healthy Program is identical to the program developed by the Nasseff Heart Center at United Hospital. We are proud to be the first independent transitional care facility in the Twin Cities authorized to offer it to our post-cardiac incident patients.

 

               

 

Our rehab nurses and therapists have all undergone the same cardiac-specific training as their hospital-based peers.

 

The Heart Healthy Program provides an interdisciplinary approach to promoting your recovery, plus education and follow-up to help you live the rest of your life to the fullest. There are several keys to the success of this program:

 

Personalized Physical Therapy
Your exercise program will be designed around comprehensive interdisciplinary evaluations and your own needs and goals. It will include:

A flexible therapy schedule on state-of-the-art equipment How to breathe while exercising Signs and symptoms to watch for Developing an exercise plan for when you get home What therapy equipment you’ll need at home

 

Cardiac-specific Care and Education
Our cardiac-trained nursing team provides unsurpassed inpatient care, and will educate you and your family about how to stay Heart Healthy:

 

Your heart medications and possible side-effects Signs and symptoms of cardiac changes How to manage pain What to do and who to call in case of emergencies A personalized after-care program

 

Heart Healthy Dining
While you’re with us, you’ll discover how tasty a low-sodium (2 gram/day) diet can be as you enjoy made-to-order meals served in your room. You’ll also learn how to plan Heart Healthy meals when you get home:
