Louverne Williams was our very first transitional care patient. She chose us after hip replacement surgery back in January 2003. When her other hip was replaced in August 2006, Louverne chose us again.

- Our transitional care program is Medicare/Medicaid certified and fully lives up to our 113-year reputation for exceptional nursing care

- We do assessments and admissions "24/7"
- PT, OT and ST are available seven days a week
- We accept both inpatient and outpatient referrals regardless of age, gender, race, religious affiliation, sexual orientation, national origin or handicap
